What documents are needed for KEIL Affiliation of a private dental clinic

Kerala Enviro Infrastructure  Ltd ( KEIL) is the only Hazardous Waste Management facility in Kerala. 

Inorder to affiliate with KEIL 

Login in to www.keralaenviro.com

Select- Downloads 

Then click on - Affiliation form for Dental Clinic

Next select - Private Dental Clinic 

You can directly download all the forms for Affiliation from site.

Documents that needs to be attached along with application form includes

1. Professional Registration Certificate (self attestation required)

2. PAN Card Copy (self attestation required)

3. Clinic Registration Certificate 

4.PCB copy ,if any

5.GST copy,if any

Those who already have IMAGE Affiliation submit 

6. Image Affiliation copy

7. Past 3 months image bill copy

Courier the forms and documents to office address.

After verification,Affiliation Certificate will be send to you via email .
