Dr.RAFIUDDIN AHMED - Father of Indian Dentistry


Dr.RAFIUDDIN AHMED (1890-1965) 

He is a well-known personality and great visionary in dentistry, renowned as Father of Indian Dentistry. 

He was born on December 24, 1890, in Bardhanpara, East Bengal, India.

His birthday is celebrated as NATIONAL DENTISTS DAY.




Dr.RAFIUDDIN AHMED came back to India from the US after his studies from The Dental University Of Boston.

He started practising dentistry in Kolkata in the year 1919.

He raised funds from his own practice to start the first dental college in the Asian continent in Kolkata, India in 1920.

Dr Ahmed published the First Student’s Handbook on Operative Dentistry in 1928.

 He also established the Indian Dental Journal in 1925 and was its Editor until 1946.

In 1925, Dr Ahmed established the Bengal Dental Association, which became the forerunner for the Indian Dental Association (which he also organized in 1928).

He served three terms as President of the Indian Dental Association from 1945 to 1949.

 Dr R. Ahmed was the first elected President of the India Dental Council, serving from 1954 to 1958.

 On his birthday December 24th is observed as National Dentists Day.

He died on January 18, 1965.

               Mercury usage in dentistry/Harmful effects of Dental Amalgam Filling

Mercury is used in medical instruments such as clinical thermometers and sphygmomanometers. It is also used in dentistry ,primarily in amalgam fillings.

High doses of mercury vapours escapes from amalgam fillings when ultrasonic scalers is used on amalgam surface or while polishing amalgam.

Use of hydrogen peroxide or other bleaching agents for teeth whitening  also releases mercury.

Maternal mercury vapours exposure can results in birth defects,it can affect fetus.Hence pregnant women are alert o be away from this.


Dental hygienists are advised not to polish amalgams when cleaning teeth.

Balanced Diet

BALANCED DIET

According to Center for Nutrition Policy and Promotion of USA , a balanced diet should include :

·         Fruits and Vegetables- Combined these should be half of what you eat every day.

·         Grains - At least half of the grains you eat should be whole wheat such as whole wheat bread and brown rice.

·         Dairy - low fat or fat free diary foods.

·         Lean Proteins - such as lean beef,skinless poultry, and fish . Try to vary your protein choices to include eggs, beans, peas and legumes.

Eat at least 8 oz. of sea food a week.

Main area of focus is that, food should be pesticide or chemical free foods .

Now a days most of the food items we buy from market will be adulterated ,which is the prime spot of all these diseses what we are encountering day today life,the soul reason for cancers too.

Try to cultivate resources for daily intake  by your own.

Pacifier

PACIFIER
A pacifier isn't a substitute for nurturing or feeding, of course, but if your baby is still fussy after you've fed, burped, cuddled, rocked, and played with her, you might want to see if a pacifier will satisfy her.
There's another benefit to using a pacifier: Some studies have shown that babies who use pacifiers at bedtime and nap time have a lower risk of  sudden infant death syndrome. These studies don't show that the pacifier itself prevents SIDS, just that there's a strong association between pacifier use and a lower risk of SIDS.
Also, a pacifier habit is easier to break than a thumb-sucking habit.Pacifier use may increase the risk of middle ear infections in babies and young children.
Use of pacifier during the first few years doesn't cause long-term dental problems.

However, Prolonged pacifier use might affect the shape of his mouth by misaligned teeth.

REGENERATIVE ENDODONTICS

Regenerative endodontics, is the extension of root canal therapy

Conventional root canal therapy cleans and fills the pulp chamber with biologically inert material after destruction of the pulp due to dental caries, congenital deformity or trauma.

 Regenerative endodontics , instead seeks to replace live tissue in the pulp chamber.

Regenerative endodontics uses the concept of tissue engineering to restore the root canals to a healthy state, allowing for continued development of the root and surrounding tissue. 

To replace live tissue, either the existing cells of the body are stimulated to regrow the tissue native to the area or bio active substances inserted in the pulp chamber. 

INDICATION
Tooth with open apex.i.e treatment of necrotic immature permanent teeth resulting in continued root development, increased thickness in the dentinal walls and apical closure.

PROCEDURE

Case Selection:

Tooth with necrotic pulp and an immature apex

Pulp space not needed for post/core, final restoration

No known allergies to antibiotics if intended for use

Compliant patient (parent/guardian)
Informed Consent

Two (or more) appointments

Use of antimicrobial(s)

Possible adverse effects: staining of crown/root, lack of response to treatment, pain/infection

Alternatives: MTA apexification, no treatment, extraction (when deemed non‐salvageable)

First Appointment

Local anesthesia, rubber dam isolation, access

Copious, gentle irrigation with 20ml 1.5% NaOCl using an irrigation system that minimizes the possibility of extrusion of irrigants into the periapical space (e.g., needle with closed end and side‐vents, or EndoVac). The lower concentrations of NaOCl are advised, to minimize cytotoxicity to stem cells in the apical tissues.

Dry canals

Place antibiotic paste or calcium hydroxide. Ca(OH)2 is antimicrobial at concentrations that do not induce stem cell toxicity and is widely available. As an alternative, if the triple antibiotic paste is used: 1) consider sealing pulp chamber with a dentin bonding agent [to minimize risk of staining] and 2) mix 1:1:1 ciprofloxacin:metronidazole:minocycline in a lower concentration (0.01‐0.1 mg/ml) to avoid stem cell toxicity; these lower concentrations appear as a liquid form and are no longer a paste.

Deliver into canal system via Lentulo spiral, MAP system or syringe

If triple antibiotic is used, ensure that it remains below CEJ (minimize crown staining). As an alternative, Ca(OH)2 does not cause staining.

Seal with 3‐4mm Cavit, followed by IRM, glass ionomer cement or another temporary material

Dismiss patient for 3‐4 weeks

Second Appointment

Assess response to initial treatment. If there are signs/symptoms of persistent infection, consider additional treatment with the antimicrobial, or an alternative
antimicrobial. Recall the patient in about 3‐4 weeks as before.

Anesthesia with 3% mepivacaine without vasoconstrictor, rubber dam, isolation

Copious, slow irrigation with 20ml 17% EDTA, followed by normal saline, using a similar closed end needle.

Dry with paper points

Create bleeding into canal system by over‐instrumenting (endo file, endo explorer)

Stop bleeding 3mm from CEJ

Place CollaPlug/Collacote at 3mm below CEJ.

Place 3‐4mm of a MTA and reinforced glass ionomer and place permanent restoration. Glass ionomer may be an alternative to MTA in cases where discoloration of the crown is a potential concern.

Follow‐up

Clinical and Radiographic exam:
o
No pain or soft tissue swelling (often observed between first and second appointments)
o
Resolution of apical radiolucency (often observed 6‐12 months after treatment)
o
Increased width of root walls (this is generally observed before apparent increase in root length and often occurs 12‐24 months after treatment)
o
Increased root length
o

* Look for Kid friendly toothpastes that are fruity,gummy flavour and a  flouride containing toothpaste rather than adult toothpaste.

It is the easiest way of brushing. One can operate the brush by switching it on and no need to master the technique to brush.



Power brushes remove significantly more plaque and improve gum health better than a manual toothbrush.

Re commented for all age groups especially medically compromised patients , children .

But the efficiency of cleaning in the inter dental area is questionable .

Power brushes are available in various types and forms ,but its costly.

BRUSHING TECHNIQUES

There are various mechanical methods of tooth brushing

First and foremost thing is to identify the purpose of brushing habit.It as simple as to wash away the particles that sticks in the oral cavity, mainly the food particles.


  • So how long to brush?


Just 2 minutes .


  • How much pressure you should give while brushing?


Just to wipe off the food materials,so only as gentle without damaging teeth.


  • Which type of brush?


Always use a SOFT bristle toothbrush


  • TECHNIQUE


Proper understanding of surfaces of a tooth helps in proper brushing.


  • how many surface a tooth has?

  • Always follow a vertical pattern of brushing in front areas of teeth ,as horizontal method is not advisable.
  • Horizontal method may result in abrasion of teeth especially in the gum lines.

  • There comes the need to FLOSS, use of  INTERDENTAL TOOTH BRUSH .



  • Always many people tends to miss cleaning of last teeth properly and  find it difficult to do.

Loss of teeth is linked for cognitive impairement and dementia


 According to researchers, loss of teeth is linked to increased risk for cognitive impairment and dementia in adults.

we are often ignorant about loss of teeth but preserving oral health can benefit you in a longer run.

The findings showed that individuals with less than 20 teeth were at a 20 percent higher risk for developing cognitive decline and dementia than those with greater than or equal to 20 teeth.

JACK FRUIT

Jack fruits are used during the season as a fruit or as a vegetable ingredient for a several traditional 
Indian dishes.

A Jackfruit tree extends a human life by 
10yrs with just 60 days of fruits per year”

 During off season, it is used as fried or dried chips, or as a canned fruit. 

Only Sri Lanka and Kerala uses mature unripe jack fruit to prepare a whole meal as an alternate to Rice or Wheat which means higher quantity of consumption. Rest of the world uses tender jack fruit as a side dish in low quantity per meal.

Jack fruit has a very short season of 2-3 months .

 Most nutritional values of jack fruits are for Ripe jack fruit which is high in sugar hence many overlooked unripe jack fruit.hence never considered as a viable diet.

However almost 80% of the Jack fruits in India are going waste as there is no organized market for Jack fruit based products and Chefs are reluctant to use Jack Fruit for three reasons- seasonal, messy to prepare and the aroma spreads in the kitchen.


Consumption of mature unripe jack fruit can reduce insulin dependency on patients due to:
1. Low glycemic index
2. High dietary fiber especially insoluble fiber
3. Possible to eat large quantity as a meal and satiate a hungry diabetic patient
In addition jack fruit is Gluten Free and can be a silver bullet for diabetic patients with Gluten allergy
1. Hypothesis for Hypoglycemic activity
Jackfruit meal has a low Glycemic Index (GI). The low GI could be due to the collective contributions from dietary fiber, slowly available glucose and un-gelatinised (intact) starch granules in the seeds. Since the study compared the Jack fruit meal with white bread as a standard, according to the current Glucose standards for glycemic index, the value would be 52.5 i.e. low GI.
2 . Unripe raw jackfruit has very low sugar level, only 20% sugar level Vs ripe pods.
